Life Care Planning 101


Role of the Life Care Planner

The role of the life care planner involves the coordination of the home evaluation, serving as patient advocate, comprehensive review of all available medical records, consultation with all treating health care providers and or educators, development of the life care plan and presentation of the life care plan as indicated.


Components of the Life Care Plan
Components of the life care plan include a comprehensive chronology/history documented in the medical records, a nursing assessment, cost projections consisting of one or more alternatives for future care, a cost projection summary utilizing U. S. Vital Statistic Life Tables for life expectancy date, a complete list of records reviewed and a resource directory of providers educators, and vendors consulted in calculating the frequency and cost of services. The completed life care planmay or may not be shared with the client at the discretion of the requesting individuals/corporation. When funding is available Nursing Consultation Services, Ltd. is available to provide life care management services and or interim consultation to assist the client to secure the services and outcomes identified in the life care plan inclusive of:
• assessing architectural barriers and equipment needs
• addressing future educational/vocational needs and identifying family education and support


Participants in the Life Care Planning Process
The client and family are the primary participants in the development of the plan. During the
home evaluation process both client and family interact with the life care planner to provide
an accurate reflection of the comprehensive care needs of the individual. The life care planner
then proceeds to develop a nursing assessment and outline alternatives of care with input from
current treating health care providers.
